other گزارش دو مورد اقدام به خودکشی با مصرف بیش از حد دارو در بیماران افسرده بستری در بخش روانپزشکی: چالش‌های ایمنی بیمار و درس‌های آموخته Report of Two Cases of Drug Overdose Suicide Attempts in Depressed Patients Hospitalized in a Psychiatric Ward: Patient Safety Challenges and Lessons Learned عمومى General گزارش مورد case report زمینه و هدف: بیماران مبتلا به اختلال افسردگی اساسی بستری در بخش‌های روانپزشکی، به‌ویژه افراد دارای سابقه اقدام به خودکشی و سوءمصرف مواد، در معرض خطر بالای رفتارهای خودآسیب‌رسان قرار دارند. علی‌رغم اجرای پروتکل‌های استاندارد پیشگیری از خودکشی، دسترسی غیرمجاز بیماران به داروهای روان‌گردان و مصرف خودسرانه آن‌ها همچنان یکی از چالش‌های مهم ایمنی بیمار در این بخش‌هاست. هدف از این گزارش، ارائه دو مورد اقدام به خودکشی با مصرف بیش از حد دارو و بررسی درس‌آموخته‌های مرتبط با ارتقای ایمنی بیماران است. گزارش مورد: دو بیمار زن ۳۷ و ۲۹ ساله، با تشخیص اختلال افسردگی اساسی، سابقه اقدام به خودکشی و سوءمصرف مواد، در بخش روانپزشکی بستری شدند. برای هر دو بیمار درمان دارویی ترکیبی تجویز و دستورات پزشکی شامل نظارت دقیق، کنترل وسایل تیز و برنده و جلوگیری از مصرف خودسرانه دارو صادر شد. با این حال، در چهاردهمین روز از بستری بیمار اول و یازدهمین روز از بستری بیمار دوم، هر دو بیمار با مصرف پنج عدد قرص کلونازپام ۱ میلی‌گرمی و پنج عدد قرص آلپرازولام 5/0 میلی‌گرمی که به‌صورت غیرمجاز وارد بخش شده بود، اقدام به خودکشی نمودند.  هر دو بیمار طی چند دقیقه پس از مصرف، پرستار را مطلع کردند. اقدامات فوری شامل اطلاع پزشک معالج، شست‌وشوی معده از طریق لوله بینی- معده، سرم‌درمانی، پایش مستمر علائم حیاتی و مشاوره با مرکز مسمومیت انجام شد. وضعیت عمومی بیماران پس از مداخلات درمانی سریع پایدار شد و با هوشیاری کامل به شیفت بعد تحویل داده شدند. نتیجه‌گیری: این موارد نشان می‌دهد که صرف صدور دستورات نظارتی مکتوب برای پیشگیری از دسترسی بیماران پرخطر به دارو کافی نیست. تقویت نظارت پرستاری، کنترل دقیق ورود و توزیع داروها، بررسی دوره‌ای وسایل شخصی بیماران، استفاده از چک‌لیست‌های استاندارد ایمنی و ایجاد محیط درمانی امن‌تر می‌تواند در کاهش خطر تکرار چنین رویدادهایی مؤثر باشد.   Background and Objectives: Patients with major depressive disorder admitted to psychiatric wards, particularly those with a history of suicide attempts and substance abuse, are at high risk of self-harm behaviors. Despite the implementation of standard suicide prevention protocols, patients’ unauthorized access to psychotropic medications and unsupervised medication use, remain important patient safety challenges in psychiatric settings. This report presents two cases of suicide attempts involving medication overdose and discusses lessons learned to improve patient safety. Case Report: Two female patients, aged 37 and 29 years, with major depressive disorder, a history of suicide attempts, and substance abuse, were admitted to a psychiatric ward. Both patients received combination pharmacotherapy, and medical orders included close monitoring, control of sharp objects, and prevention of unsupervised medication use. However, on the 14th day of hospitalization for the first patient and the 11th day for the second, both patients attempted suicide by ingesting five 1-mg clonazepam tablets and five 0.5-mg alprazolam tablets that had been brought into the ward without authorization. Both patients informed the nurse within a few minutes of ingestion. Immediate interventions included notifying the attending physician, gastric lavage via a nasogastric tube, intravenous fluid therapy, continuous monitoring of vital signs, and consultation with the Poison Control Center. Following prompt therapeutic interventions, the general condition of both patients stabilized, and they were transferred to the next shift in full consciousness. Conclusion: These cases demonstrate that written monitoring orders alone are insufficient to prevent high-risk patients from accessing medications.
