fa مقایسه هزینه‌ها و کیفیت خدمات بستری قبل و پس از اجرای طرح تحول نظام سلامت تخصصي Special پژوهشي Research <table align="center" cellpadding="0" cellspacing="0" hspace="0" vspace="0"> <tbody> <tr> <td align="left"> <p dir="RTL" style="text-align: justify;"><strong>زمینه و هدف: </strong>با توجه به اهمیت سلامت و بهداشت در جامعه، تأمین منابع مالی مراقبت&shy;های بهداشتی و درمانی اهمیت زیادی دارد. برای این منظور و تأمین خدمات بهداشتی و سلامت، دولت ایران به اجرای طرح تحول نظام سلامت همت گماشت. هدف از این پژوهش مقایسه هزینه&shy;‌ها و کیفیت خدمات بستری قبل و پس از اجرای طرح تحول نظام سلامت بود.</p> <p dir="RTL" style="text-align: justify;"><strong>مواد و روش&shy;ها:</strong> در این مطالعه توصیفی-مقایسه&shy;‌ای، بیماران بستری در بیمارستان بوعلی ساری از مهر لغایت آذر 1394 که قبل از اجرای طرح تحول نظام سلامت نیز بستری شده بودند به روش تصادفی ساده انتخاب شده و مورد بررسی قرار گرفتند. گردآوری اطلاعات توسط پرسشنامه استاندارد وزارت بهداشت و پرونده&shy;های مالی بیماران انجام شد. بیماران به سؤالات پرسشنامه به صورت قبل و بعد پاسخ داده و پاسخ&shy;ها مقایسه شدند. تجزیه و تحلیل داده&shy;ها با استفاده از آزمون کولموگروف-اسمیرنوف، <span dir="LTR">t</span> و <span dir="LTR">ANOVA</span> و توسط نرم&shy;افزار آماری <span dir="LTR">SPSS</span> انجام شد.</p> <p dir="RTL" style="text-align: justify;"><strong>یافته&shy;‌ها:</strong> در مجموع 108 زن (91/63%) و 61 مرد (09/36%) در مطالعه شرکت کردند. هزینه خدمات بستری، تجهیزات، تخت، دارو و جراحی قبل از اجرای طرح کمتر بود (0001/0=<span dir="LTR">p</span>). همچنین کیفیت خدمات بستری پس از اجرای طرح افزایش یافت (004/0=<span dir="LTR">p</span>). کیفیت تجهیزات و تیم درمانی تفاوتی نداشت (05/0<<span dir="LTR">p</span>). کیفیت خدمات پذیرش، پزشکان و پرستاران، فراهم بودن دارو و ملزومات مربوط به آن پس از اجرای طرح افزایش یافته ولی کیفیت خدمات پرستاری و پرستاران و محیط فیزیکی بیمارستان کاهش یافت (05/0<<span dir="LTR">p</span>).</p> <p dir="RTL" style="text-align: justify;"><strong>نتیجه&shy;‌گیری: </strong>با توجه به یافته&shy;های پژوهش به نظر می&shy;رسد طرح تحول نظام سلامت نیاز به بازنگری اساسی و برنامه&shy;‌ریزی&shy;های دقیق&shy;تری جهت بهبود وضعیت خود دارد.</p> <p dir="RTL" style="text-align: justify;"></p> </td> </tr> </tbody> </table> <div style="clear: both; text-align: justify;"></div> <p style="text-align: justify;"></p> <p><strong>Background & Aim:</strong> Given the importance of health and hygiene in society, financing healthcare is very important. Therefore, the government of Islamic Republic of Iran codified and implemented the health reform plan. The current study aimed to compare the costs and quality of admission services before and after the implementation of the health reform plan.</p> <p><strong>Material & Methods:</strong> In this descriptive-comparison study, patients admitted to Bu-Ali Hospital of Sari from October to December 2015 who had a history of hospitalization prior to the implementation of health reform plan were randomly selected. For data collection, the standard questionnaire developed by the Ministry of Health and Medical Education and patients&rsquo; financial records were used. Patients answered the questionnaires as &ldquo;before&rdquo; and &ldquo;after&rdquo; the implementation of health reform plan and their answers were compared. Data was analysis using descriptive statistics and the Kolmogorov-Smirnov test, t-test and ANOVA using SPSS software.</p> <p><strong>Results:</strong> A total of 108 women (63.91%) and 61 men (36.09%) participated in the study. The admission, equipment and bed costs were lower before the plan (p=0.0001). Also, the quality of inpatient services increased after the plan (p=0.004). Quality of equipments and the medical team was not statistically different after the plan p>0.05). Quality of reception services, doctors and nurses, the availability of medicines and supplies increased after the implementation of plan but quality of nursing service and physical environment of the hospital reduced (p<0.05).</p> <p><strong>Conclusion:</strong> According to the study findings, it seems that the health reform plan needs to be comprehensively revised, and more accurate plans should be developed for improvement.</p> <p></p> بیمارستان, بستری, هزینه­, طرح تحول نظام سلامت.
